Coastal Vibes Counseling, LLC is a generally a self-pay practice  as we are only in network with Horizon BCBS PPO insurance plans. Should you not have this provider or type of plan and wish to pursue services at an Out Of Network fee, a session receipt or monthly superbill (with medical coding) will be provided so you can independently pursue reimbursement for services through your provider. 

I encourage you to review your current plan's "Out-Of-Network benefits". This value may be printed on the back of your card and could already be met through various appointments you attended throughout the year. To find out call the member services 1-800 number on the back of your card.
